Job Description

• Perform assessment of clients' physical mobility and falls risk
• Develop and implement care plans for clients and their care-givers with other clinical staff
• Provide appropriate physiotherapy interventions and based on assessment of clients
• Conduct functional mobility training including the prescription and use of appropriate mobility/walking aids
• Plan, implement and evaluate individual and group physical activity and exercise programs
• Conduct and/or supervise group therapy and exercises for the clients
• Conduct education and training for care-givers
• Maintain close communication on the clients' progress in the centre
• Conduct in-service and on-the-job training sessions for staff and volunteers

Requirements

• Registered with Allied Health Professions Council and Full Registration
• Relevant Diploma/Degree in Physiotherapy
• At least 2 years working experience
• Interested in rehabilitative and geriatric care and enjoys working with the elderly
• Maintain documentation of clients' progress and outcomes of care provision
• Supervise Therapy Aides/Assistants in the provision of therapy services to clients
• Cheerful disposition with good communication skills

Role & Responsibilities:

• Assist PT/OT in conducting rehab therapy sessions in Nursing Home/ Centre-Care Based
• Conduct maintenance exercise / recreation activities to residence of Nursing Home or Clients in Centre-Care Based
• Play as team members to deliver holistic Person- Centre-Care to residents/clients
• Play important role in care planning, implementation, revision to individual residents / clients

Requirements:

• Minimum GCE O Level
• Preferable with Certificate in Therapy Services
• Prior Healthcare background would be a strong advantage

Job Description

• Responsible for the safe delivery of care and clinical decisions to all residents
• Render clinical care and nursing procedures, including proper assessment of residents, administration of medications, and injections, performing complex wound dressings and monitoring of vital signs.
• Responsible for the ordering, packing and preparation of the daily medications for all the residents

Requirement

- Diploma in Nursing

- Registered Nurse with the Singapore Nursing Board (SNB).

- Relevant post-basic qualification

- Interested in rehabilitative and geriatric care and enjoys working with the elderly

- At least 1 year of working experience

- Pleasant and cheerful disposition with good communication skills

Job Description

• Carry out basic nursing procedures such as simple wound dressing
• Assist doctors and registered nurses in simple and moderately complex procedures/treatments under supervision
• Document, monitor and record observations of patients and their vital signs, and report changes in condition to the registered nurse promptly
• Provide simple health education to patients and families
• Participate in clinical/service quality improvement activities or nursing research, contribute to changes/improvement of workflow
