RESPONSIBILITIES

Casework - Engagement & Assessment

• Engage client and significant others (e.g. family, caregivers, etc.) in a therapeutic manner to understand their needs
• Manage and overcome reluctance and resistance to change

Casework - Case Review & Documentations

• Implement and follow up on care/ case plans and interventions to determine the suitability of care/ case plans in addressing client's needs and modify them where necessary
• Draft social reports for complex cases to be reviewed by supervisor

Casework - Goal Setting & Intervention Plans

• Conceptualise and identify appropriate approaches to develop care/ case plan for client
• Help clients to develop and choose strategies and formulate viable plans
• Represent case in case conference

Group Work

• Target and reach out to individuals and/ or targeted groups who may benefit from group work conducted by organisations/ agencies

Community Work

• Conduct assessment, planning and intervention for cases
• Participate in internal and external case conferences, networking and case management meetings
• Document and evaluate case outcomes

Information and Referral

• Provide accurate information and refer clients to appropriate agencies for further intervention
• Attends to walk-in clients
• Adhere to CSWP and SSNet processes in managing referral

Programme – Groupwork / Community Work

• To provide support to Coordinator in planning, implementation, evaluation and documentation of the programme
• To plan and organise events and activities for clients

Qualifications and Requirements

• Degree or Post-Graduate Diploma in Social Work
• Min 3-5 years relevant work experience in Social Work setting
• Demonstrate ability to handle work professionally, independently and efficiently with a strong customer service mindset
• Possess effective organizational and follow-up skills, paying attention to detail
• Good interpersonal skills and able to interface with all levels, internally and externally, in a professional and responsive manner.

Position: Youth Care Worker
Salary: $2,200 to $3,300
Working Days: 5 days (rotating night shift)
Location: Potong Pasir

Responsibilities

• Carry out the daily operational duties and supervise residents in their regular activities
• Assist residents with any difficulties related to day-to-day issues and ensure their welfare and safety
• Promote discipline among the residents and be a mentor/role model to them
• Assess, respond and handle crises or conflicts
• Carry out individual therapeutic interventions based on care plans by treatment team
• Participate in providing physical, emotional, spiritual, mental, educational, and social needs of residents

Requirement 

• With experience in working with youth at-risk
• With basic processing and counselling skills
• With knowledge of common issues and disorders that appears in trauma and abuse
• Willing to work on 3-shift rotations and over weekend/Public Holidays
