You will be responsible for:

• Supporting the day-to-day operations of the Lodge
• Managing facilities maintenance, repairs, and improvement works

• Obtaining quotations, managing procurement and tracking operational budgets

About Apex Harmony Lodge
Founded in 1999, Apex Harmony Lodge (The Lodge) is a 210-bed nursing home dedicated to providing specialized and person-centred care for individuals living with dementia. We strive to be a leader in dementia care, setting benchmarks in clinical excellence, workforce capability building, and system innovation.

The Physiotherapist will be responsible for leading and implementing rehabilitation programs at Apex Harmony Lodge. Duties must be carried out in adherence to professional ethics, statutory regulations, departmental standards, and institutional values. The incumbent is expected to fully support the Lodge’s mission, vision, and values, demonstrate sound knowledge of healthcare systems, and effectively collaborate with a diverse workforce to enhance residents’ quality of life.

The incumbent will provide leadership and guidance to staff in delivering comprehensive rehabilitation services and programs, while actively participating in departmental and institutional initiatives that contribute to holistic resident care.

Key Responsibilities

Assessment and Evaluation

• Conduct thorough assessments of residents’ physical, emotional, and social needs to develop individualized treatment plans aligned with therapeutic goals.

Treatment Planning

• Design and implement rehabilitation programs, including physical exercises, therapeutic activities, and interventions to improve functional abilities and overall well-being.

Therapeutic Interventions

• Apply evidence-based techniques such as exercise therapy, heat treatment, and hydrotherapy to support recovery, relieve pain, and enhance mobility.

Progress Monitoring

• Track and document residents’ progress, review and adjust treatment plans as necessary to ensure effective outcomes.

Education

• Resident & Staff Development: Provide education on exercise regimes, therapeutic techniques, and wellness practices to enhance functional, psychosocial, and emotional well-being.
• Therapy Assistant Training: Lead induction, clinical education, and in-service training for therapy assistants. Support unit- and core-based professional growth while serving as a preceptor and mentor.
• Program Innovation: Establish, expand, and design innovative learning opportunities to strengthen and upgrade therapy assistants’ skills and capabilities.

Collaboration

• Work closely within multidisciplinary team, which include physiotherapist aides, exercise therapists, psychologists, nurses and other healthcare professionals to deliver integrated care and achieve best rehabilitation outcomes for residents.

Engagement

• Actively participate and contribute in departmental & institutional overaching programs, events, committees, and workgroups etc.

Administrative Duties

• Maintain accurate resident records, schedule staff members’ duties equitably, prepare required reports, and ensure compliance with healthcare regulations (e.g., InterRAI, Healthcare Service Act).

Job Requirements

• Bachelor’s degree in Physiotherapy.
• 3 – 5 years of professional experience as a Physiotherapist or Occupational Therapist.
• Registered with the Allied Health Professions Council and holder of a valid Practising Certificate (PC).
• Experience in eldercare and dementia care, with the ability to integrate evidence-based interventions into nursing practice.
• Prior experience in teaching, curriculum development, or capability building in nursing will be advantageous.

We are seeking a highly motivated, compassionate and dedicated Case Manager to join our team in providing holistic, person-centred care for residents living with dementia. Key Responsibilities:

• Coordinate and oversee individualized care plans for residents with dementia
• Work closely with a multidisciplinary team (nurses, therapists, doctors, social workers) to ensure quality care. Participate in team meetings
• Serve as the main point of contact for residents’ families, providing updates and support
• Monitor residents’ progress and adjust care plans as needed
• Facilitate admissions, care transitions, and discharge planning
• Assist the Medical Social Worker with managing workload, conducting audits, preparing reports, and implementing quality improvement initiatives related to social care coordination

The role requires attention to detail, integrity, and sensitivity to the unique environment of a care facility serving residents with dementia.

1. Accounts Payable & Receivable

• Process invoices, verify accuracy, and ensure timely payments to vendors
• Prepare and issue invoices to residents, families, or relevant agencies
• Monitor outstanding balances and follow up on payments
• Maintain proper documentation and filing of all financial transactions

2. Financial Record Keeping

• Maintain accurate and up-to-date accounting records
• Assist in reconciling bank statements and petty cash
