• Lead the work with MOH and relevant stakeholders to develop and implement plans as well as build partnerships within Community Care Sector to achieve coordinated care for the seniors and anchor care within the community. This includes the funding design and how to create incentives that drive desired behaviours/outcomes, and to develop evaluation plan.
• Drive the transformation of sector which includes the planning and conceptualisation of the future Community Care Sector
• Establish and manage team and partnerships with relevant stakeholders for data collection, tracking and analysis to recommend initiatives/measures to support the needs and development of the Community Care Sector
• Contribute to the overall planning and development of the key workstreams under the Programme Office, and to ensure alignment with policy objectives
• Support secretariat duties for key meeting platforms
   

Job Requirements

• A recognised degree with at least 8 years of relevant experience, preferably with prior experience in strategy/policy development and/or community care sector, or familiar with national healthcare/long-term care related policies.

The job scope includes

• Develop comprehensive plans to strategically locate community care facilities across the island, and creatively utilising innovative space solutions to address land scarcity, ensuring optimal accessibility and service delivery for our senior population.
• Conduct comprehensive site assessments to determine the suitability of locations for community care facilities.
• Analyse demographic data and community needs to inform the planning and assessment decisions.
• Collaborate with diverse stakeholders to co-design and co-develop functional community care facilities that enhance the delivery of care services for the seniors.
• Assist in drafting compelling papers, reports, presentations to secure approvals from management and the Ministry.

Job Requirements:

You will drive strategic initiatives through the Community Silver Trust (CST), assisting eligible community care organisations optimise their donations. By collaborating closely with partners, you will guide the effective use of matched funds to enhance services, build capabilities, and address emerging healthcare challenges in alignment with national priorities for seniors and the broader population. 

• Evaluate funding proposals and and guide eligible community care organisations in developing strategic initiatives that maximise their matched funding opportunities.
• Monitor project implementation and outcomes, ensuring alignment with funding guidelines while supporting partners to enhance service impact.
• Work with community care partners to identify service enhancement opportunities and develop proposals that address emerging needs.

As Senior Executive, Partners Development and Account Management, you will be part of a team that is working to build an integrated and resilient care ecosystem that enables and empowers seniors to age in place.  You will support the operationalisation of projects and initiatives that will help promote collaboration and partnerships amongst different stakeholders in the community care sector that will work together to support our seniors. You will also develop enablers and resources to equip and support AIC’s Account Managers to better engage community care providers and other non-care partners towards achieving AIC’s goals and outcomes. 

• Develop, implement, monitor and review enablers that will enable building of strong collaboration and partnerships in the community care sector
• Identify best-practices, plan and organise engagement sessions, and cross sharing and learnings platforms with providers
• Distill and synthesize the learnings and experience of ongoing partnerships through regular engagements to enable the improvement of existing resources; and development of new enablers.

As an Assistant Manager/ Senior Executive, you will be part of the team in supporting the development and implementation of Advanced Care Planning and the Palliative Care sector in Singapore:

• Work with policy makers, community stakeholders, including service providers and hospitals to drive the development and implementation of the ACP programme; development and implementation of the National Strategy for Palliative Care including involvement in palliative care related pilot programmes, service models and funding policies that support, management of capability and capacity and enablers in the ecosystem.

As an Account Manager, you will build strong relationships and trust with our sector partners,  Partners’ capabilities, deepen their impact and facilitate strategic alignment with MOH and AIC.

• Analyse and review consolidated performance data from all Active Ageing Centres (AACs) to evaluate outcomes, identify trends, and manage transitions from programme changes, and ensure timely resolution of related appeals with proper approval framework and data governance
• Assist in developing and customising comprehensive performance reports for senior leadership and stakeholders, to facilitate sector engagements and programme review for improvements
• Collaborate closely with stakeholders to refine and enhance reporting templates in line with programme changes and translate these requirements into IT system specifications including participating in User Acceptance Testing (UAT)
• Engage with service providers to gather insights and best practices, supporting policy formulation and programme development
• Assist in the integration and streamlining of diverse data sources to support monitoring and process development, enhancing the accuracy and efficiency of programme reporting and development

To ensure clients’ enquiries, escalations and appeals for the long-term care schemes are responded and managed in a timely and client-centric manner

• Handle enquiries and coordinate with stakeholders to look into clients or caregivers requests and feedback to achieve closure on issues raised

You will handle outreach engagement, volunteer management and stakeholder management to sense needs of seniors and support them on their ageing needs. By working collaboratively with volunteers and various agencies and community partners, you will identify, analyse and render support for seniors to age in place through care coordination and referrals. 

Client Engagement and Service Operations

• Operationalise engagement strategies through various outreach modalities and ensure optimal deployment of volunteers - Silver Generation Ambassadors (SGAs) 
• Sense needs and identify older adults who require additional care and support and to coach SGAs on the same, and consolidate feedback from older adults and partners to track fulfilment, identify and analyse needs in the community
• Ensure that stipulated performance standards are met and execute operation validation as required and ensure engagement quality and outcomes are maintained
• Coordinate active ageing programmes and functional screenings with community partners and government agencies to ensure events are held smoothly and divisional KPIs are met
   

Volunteer Management

• Supervise and motivate a team of SGAs to engage older adults through house visits & community events to attain the relevant desired outcomes
• Recruit, train and retain SGAs and guide them to meet the stipulated performance standards
   

Care Coordination and Community Engagement 

• Coordinate care and support needs and referrals across the relevant agencies and community partners in a seamless manner
• Build relationship with local community stakeholders and partners to identify opportunities for collaboration that are in line with divisional work-plan, and support local community pilots and community partners in the implementation of the AAC Centre model/other initiatives

You will manage the finance, procurement and office administration for the Cluster and Satellite Offices. You will also support in the curation of future volunteer training programmes/ materials, team building and learning initiatives at the Cluster level.

Finance & Procurement

• Monitor cluster budget and expenditures, assist in forecasting and manage the allocations to Satellite Offices (SOs), and highlight budget exceptions for intervention
• Collaborate with the SOs on budgetary marksmanship and strengthen projection capabilities to ensure healthy cashflow
• Manage centralized repository of finance and procurement processes, such as the procurement of goods, services and other service & supply contracts, vendor management, coordination of payments and deliveries, and distribution of items/collaterals purchased by or attained by HQ for the SOs within the Cluster
• Establish strong working relationship with the HQ F&P business partner to optimize and improve processes for more effective operations
  

Office Management & Administration

• Oversee and manage office equipment and infrastructure planning for the Cluster and SOs
• Work closely with HQ departments to coordinate volunteer management and training, gather feedback and ground sensing and identify areas for improvement, training gaps and needs

You will be part of the Care Protocol workstream that supports the development of care protocols and coordination of policy reviews for enablers, such as IT systems and applications, care support and financing capabilities. Your work will require close engagement with GPs, Primary Care Networks (PCNs) and healthcare Clusters, Hospital specialists and other agencies of MOH.  The work will involve understanding current and developing new workflows for successful development and updates of HealthierSG care protocols and safe channels of communication between primary care and hospital doctors for better patient care.

• Develop comprehensive timelines for the development, launch, and revision of all care protocols under the Healthier SG initiative.
• Execute and operationalise the established timelines, ensuring adherence to deadlines and milestones throughout the protocol development process.
• Prepare and/or review supporting materials for the development and implementation of care protocols, including but not limited to training materials and public communication documents, ensuring alignment with protocol objectives and requirements.
• Facilitate seamless coordination among internal and external stakeholders involved in drafting the care protocols and addressing implementation-related issues.
