The Assistant Nurse Manager (HPC & HPC+) supports the Nurse Manager in providing clinical leadership, governance and operational oversight for Home Personal Care (HPC) and Enhanced Home Personal Care (HPC+) services. The role combines hands-on community nursing practice with team supervision and service coordination, ensuring safe, ethical and person-centred care for clients with complex functional, medical and end-of-life needs in the community.

JOB RESPONSIBILITIES

1. Clinical Care and Service Delivery

• Provide direct nursing care and clinical oversight for clients receiving HPC and HPC+ services, including those with complex care and end-of-life needs
