 Perform Systems Administration

• Maintain Windows servers, virtualization platforms, and storage solutions.

Job Requirements

• Bachelor’s degree in information technology or related field.

• With networking, system and security certifications such as CCNA, CCNP, CCIE, Microsoft certified Azure Solutions Architect and/or Windows Server Administrator, CISSP, CISM and CompTIA Security.

• At least 5 years of experience providing systems, network and cybersecurity operational support.

• Must have hands-on experience with servers, network and security devices, firewall, switches, routers, load balancers and wireless solutions.

• Within experience in at least 5 of the below technologies:

  • Windows Server environments, including installation, configuration, maintenance, and troubleshooting.

  • Managing and maintaining Active Directory, including user and group management, Group Policy, and domain services.

  • Nutanix Hyper-Converged Infrastructure (HCI) or equivalent technologies, including deployment, configuration, and management of Nutanix clusters.

You will be working closely with the Nurse/ Care Manager to provide safe, cost effective and quality nursing care to patients in accordance to policies and procedures, standards of nursing care, professional competencies and practices to achieve the desired patient outcomes. He/she should practice within enhanced levels of clinical competence and decision-making skills whilst being imbued with clinical reasoning and critical thinking skills that promotes safety of patient and the team members. • Implements patient care in collaboration with the multi-disciplinary teams (MDT) and according to assessed needs and health condition(s).
• Facilitates admissions, transfers and discharges; ensures discharge planning is done timely upon patient’s admission and appropriate to condition.

Key Responsibilities

Clinical Assessment & Care Planning
• Conduct comprehensive admission and periodic assessments using standardized tools (e.g., Modified FRAT, FAST).
• Develop, implement, and update individualized care plans via the Elder &Community Care suite.
• Perform critical reassessments following hospitalizations, falls, or significant absences.

Medication & Clinical Management
• Oversee medication management, including preparation, packing, and administration.
• Ensure strict adherence to the IDDSI framework for residents with swallowing difficulties (dysphagia).

The Executive Assistant ensures the provision of administrative and clerical support for effective and efficient operations of the Psychosocial Services department.

• Collate and submit timely updates and reports to relevant committees, authorities or agencies.
• Responsible for all office systems, equipment, supplies and stationeries, including procurement, maintenance, repairs and stock level.
• Assist in coordinating and providing secretarial support for financial assistance/departmental/other meetings.
• Assisting to conduct means test reviews by making all necessary preparations to initiate and complete the exercise. The Care Associate (CA) will provide home-based personal care services which aims to assist clients with their daily activities of living, to enable them to be taken care of and live in the comfort and security of their own home.

Duties include but not limited to companionship, personal hygiene, medical escort, grocery shopping, meal preparation, feeding and light housekeeping.

Responsibilities

• Assist with instrumental activities of daily living (“IADLs”), including light housekeeping and laundry, assistance with medication and simple errands such as grocery shopping.

Assist in supervising pharmacy operations and provide advanced technical support to ensure medication safety, workflow efficiency, and regulatory compliance. Deliver specialized counselling and training in advanced pharmacotherapy, focusing on chronic disease management and medication safety.

Responsibilities

• Coordinate daily pharmacy workflow to ensure timely medication supply and accurate processing of prescriptions in compliance with legal requirements
• Manage and resolve medication issues with prescribers, escalating to seniors when necessary
• Ensure repackaging and compounding comply with the Healthcare Products Act
• Verify medication condition, quantity, packaging, and labeling meet prescriber intentions and legal standards

Manage referrals from AIC and direct referrals from community partners and general public. Provide end-to-end case management support and coordination to ensure a smooth admission process and service delivery for home care clients that require Home Personal Care, Home Nursing, Home Therapy, Home Medical and Medical Escort services, etc.

Client Management

• ­Manage home care client data base to ensure completeness and accuracy
• Process home care referrals through AIC referral system as point of liaison
• Perform basic financial assessment for clients or next-of-kin on home care service(s) required and refer to social worker if financial assistance is required
• Perform basic assessment of care needs for home care clients and conduct home visits where required
• Coordinate with volunteers to engage home care clients meaningfully
