Clinical Aspects

• Manage enquiries and referrals

RESPONSIBILITIES

The Crisis Operations Worker is the designated case owner responsible for the end-to-end management of child protection crisis, from the point of intake and activation through to stabilisation and transition.

This role delivers timely, safety-critical interventions to Children and Young Persons (CYP) and their families who are experiencing acute or escalating risk. It requires the integration of structured professional judgement, real-time operational control, and coordinated multi-agency response to secure immediate safety and contain risk.

Crisis Intake, Triage and Activation

• Respond and manage a crisis timely. This includes conducting urgent triage, assessing the immediacy and severity of risk, and determining the most appropriate response approach

The Senior Social Worker (SSW) is responsible for facilitating change and improving the lives of individuals, families, community facing transitional challenges through the provision of direct services.

SSW will act professionally and be accountable to both the client and the organization to ensure that intervention plans are developed and implemented based on professional judgment and knowledge. Furthermore, SSW will supervise social workers and case workers to ensure that services are delivered in accordance with organizational standards. SSW will also be in responsible for handling high-risk client cases.

RESPONSIBILITIES / MAJOR ACTIVITIES:

• Supervise and approve case assessment and outcome plans, and case closure
• Provide consultations for assessment of cases with risk issues
• Support or co-lead review of complex and risk cases in the company
• Supervise subordinates in the follow through of intervention plans, especially for complex and or risk cases
• Provide supervision of groupwork and community work through modelling, observations and debrief etc
• Function as a Team Lead or Covering Team Lead in Mental Health Service
• Ensure KPIs will be met within the timeline as per funding agencies' requirement
• Carry out any other duties as assigned by HOD and Supervisor

• Possess Degree in Social Work or Degree with a Graduate Diploma in Social Work from an accredited institution

RELEVANT EXPERIENCE:

• Accredited as Registered Social Worker with Singapore Association of Social Workers
• At least 4 years or more working experience as a Social Worker
• At least 2 years' experience in managing crisis intervention and supervising the junior social workers

RESPONSIBILITIES

The Social Work Associate (SWA) in the Central Triage Unit (CTU) provides frontline support in client enquiries, intake processes, and documentation. The SWA works closely with Social Workers and Senior-On-Duty (SOD) to ensure clients are attended to efficiently, risk concerns are flagged appropriately, and operational workflows are executed smoothly.

Job Responsibilities:

• Receive and document client enquiries via phone, email, or walk-in, and conduct initial information gathering to understand presenting issues, service needs, and urgency
• Flag risk and safety concerns (e.g., abuse, self-harm, harm to others) to SOD or other seniors immediately
• Provide basic service navigation information (e.g., community resources, helplines) to clients
• Input enquiry and intake details into SSNet and internal databases accurately and within agreed timelines
• Maintain confidential and accurate records of client interactions
• Assist in tracking turnaround times, caseload statistics, and other CTU performance metrics
• Support the CTU team in refining workflows, templates, and client-facing resources
• Participate in team discussions, case reviews, and reflective practice sessions to improve service delivery
• Provide feedback from frontline experience to enhance client journey and reduce duplication for downstream teams

• Possess Diploma in Social Work or other relevant qualifications from an accredited institution

• 1-2 years prior experience in Social Service Sector

The Services required are set out as follows:

1. To engage with identified students on a regular basis over a

prolonged duration and provide positive mentoring and/or counselling

support for students and their families.

2. To connect with identified students, build positive relationships and

contribute to their holistic development and positive school outcomes.

3. To plan and conduct student-centric interventions, including student

outreach, learning journeys, home visits and/or family engagement

sessions where necessary, customised to students' needs and aligned

with the School's efforts.

4. To plan and facilitate group-based programmes targeting students

with identified social, emotional, or behavioural needs, in consultation

with the School's student support team.

5. To observe and identify key needs of students and to report such

cases to the Student Management Key Personnel and/or School

Leaders for further intervention and support.

6. To attend case review and case management meetings with relevant

school personnel, including the Student Welfare Officer (SWO), Special

Educational Needs Officers (SENOs), and school counsellors, to provide

feedback on students' needs, programme experience, and concerns, so

that the School can better support the students.

7. To facilitate timely referrals of students and families to relevant

external agencies and community resources where additional support is

required beyond the School's capacity.

8. To maintain accurate and up-to-date case records and

documentation, and to provide regular progress updates to the School's

designated contact person.

Job Summary:

Social Workers in TOUCH Family Support play a crucial role in enabling low-income families by (1) working directly with clients to address the immediate and long-term issues they face and (2) working with the surrounding community to better support the underprivileged.

Main Responsibilities:

• Conduct intake and risk assessments for cases.
• Provide Case Management services to clients from low-income families and/or disadvantaged backgrounds
• Complete case documentation(e.g. case notes, update database, etc.) accurately and promptly.
• Developing, implementing, maintaining and evaluating programmes that meet client’s needs.
• Conduct Group work.
• Community outreach to and engagement of low-income families.
• Network and work collaboratively with stakeholders and community partners.
• Research to build domain-specific knowledge.

Working Conditions:

• 5 day work week: Mon-Fri
• Able to work on weekends when required.
• Location - North / Northeast

Essential Skills and Qualifications:

• Bachelor or Postgraduate Diploma in Social Work or relevant qualification.
• Registered Social Worker preferred with minimum 2 years of case work experience.
• Trained in usage of CSWP preferred (BPSS and FAST tool assessment).
• Good communication, interpersonal and facilitation skills.
• Excellent team player with good interpersonal skills.
