DUTIES & RESPONSIBILITIES

Reporting to the Asst /Divisional Director of Elderly Services, the Lead/Senior Centre Manager is responsible for overseeing the Active Ageing Centres.

• Curate and implement new and innovative programmes that promote active ageing and ageing well in place across the assigned Centres.
• Network with critical stakeholders and sponsors to enhance community engagement and programme sponsorship for the assigned Centres.
• Direct and supervise the operations and administrative functions of the centres assigned.
• Assist and coordinate timely submissions of assigned Centres on:
  • Dashboard Statistics (monthly/quarterly)
  • Accounts Reports
  • Volunteer Management Data
  • Corporate Communications Submissions (i.e., Admin Programmes and Order of Proceedings for events)
  • Human Resource Data (i.e., Updating training attended for P-file, time-off, MC submissions, etc
• Assist Centres with the following submissions:
  • EPES or other required KPI submissions (quarterly / annually) to Funder
  • Internal and external audit reports
  • Attaining PIF standards or other standards required by Funder.
• Communicate important messages and urgent circulars from management to the assigned Centres.
• Supervise the recruitment and performance of staff within the assigned Centres.
• Conduct the performance appraisals of Centre Managers of the assigned Centres.

• Polytechnic Diploma / WSQ Diploma/ Advanced Diploma in Social Services
• At least 7 years of related experience in the social service sector, with 4 years of supervisory experience

RESPONSIBILITIES

The Reablement Care Coordinator serves as the central coordinator of the client's reablement journey, ensuring seamless integration across disciplines, programme phases and community resources. The role bridges healthcare, social care and community support systems to facilitate meaningful and sustainable outcomes for seniors.

Key Responsibilities

Care Coordination and Case Management

• Lead end-to-end coordination of the 12-week reablement pathway.
• Conduct intake triage, screening and enrolment of programme participants.
• Review Clinical Frailty Scale (CFS) levels and population health datasets to support participant selection.
• Consolidate interdisciplinary assessments into a unified reablement plan.
• Coordinate intervention schedules across multiple disciplines and stakeholders.

Outcome Monitoring and Programme Evaluation

• Monitor client progress using validated outcome measures including INTERAI, COPM and ASCOT.
• Identify barriers to progress and coordinate intervention modifications.
• Maintain accurate programme documentation and outcome records.

Multidisciplinary Team Coordination

• Facilitate case conferences and interdisciplinary reviews.
• Coordinate communication among therapists, nurses, social workers, therapy assistants and community partners.
• Ensure continuity of care throughout programme phases.

Community Reintegration and Sustainability

• Coordinate transition plans into Active Ageing Centre programmes, CARE Market initiatives, SEED programmes, volunteer opportunities and microjob pathways.
• Support development of sustainable participation plans beyond programme discharge.

Long-Term Follow-Up

• Conduct 6-month and 12-month follow-up reviews.
• Monitor for early signs of decline and facilitate timely reactivation of services where required.

• Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy or other relevant healthcare discipline.
• Minimum 2 years of experience in healthcare, community care or case management settings.
• Strong stakeholder management and communication skills.
• Experience coordinating care across multiple service providers.
• Proficiency in Microsoft Office applications
• Experience in community care, transitional care or active ageing programmes.
• Familiarity with INTERAI, COPM, ASCOT or equivalent outcome measures.
• Experience in programme development and service improvement initiatives.
• Training in Motivational Interviewing or health coaching.
• Comfortable conducting home visits and community-based interventions

RESPONSIBILITIES

The Operations Support Assistant supports clients/ student care, cleaning, school operations, events, and basic administrative tasks within Fernvale Gardens School.

Key Tasks:

• Clean students who soil themselves
• Clean soiled area -

Casework - Engagement & Assessment

• Engage client and significant others (e.g. family, caregivers, etc.) in a therapeutic manner to understand their needs
• Manage and overcome reluctance and resistance to change

Casework - Case Review & Documentations

• Implement and follow up on care/ case plans and interventions to determine the suitability of care/ case plans in addressing client's needs and modify them where necessary
• Draft social reports for complex cases to be reviewed by supervisor

Casework - Goal Setting & Intervention Plans

• Conceptualise and identify appropriate approaches to develop care/ case plan for client

Group Work

• Target and reach out to individuals and/ or targeted groups who may benefit from group work conducted by organisations/ agencies

• Prior supervisory or leadership experience preferred
