JOB PURPOSE

You will be part of our community outreach team, also known as the Community Resource, Engagement and Support Team (CREST), focusing on raising public awareness of mental health conditions and dementia, early identification of at-risk individuals, providing emotional support to individuals and their caregivers, and linking individuals to relevant health and social care services where necessary. KEY RESPONSIBILITIES

Case Management

• Provide case management services across assessment, systems work, case conferences, and closure, and maintain proper and up-to-date case documentation
• Attend to enquiries and provide information and referral services
• Conduct basic emotional support for clients in need

Clinical Group Work

• Plan and facilitate group interventions, including case conferences with significant others and relevant partners

Outreach & Programme Planning

• Plan, coordinate, execute, and evaluate ongoing programmes and services, including outreach, on-site and online public education

QUALIFICATIONS
