RESPONSIBILITIES

The Assistant Nurse Manager (HPC & HPC+) supports the Nurse Manager in providing clinical leadership, governance and operational oversight for Home Personal Care (HPC) and Enhanced Home Personal Care (HPC+) services. The role combines hands-on community nursing practice with team supervision and service coordination, ensuring safe, ethical and person-centred care for clients with complex functional, medical and end-of-life needs in the community.

JOB RESPONSIBILITIES

1. Clinical Care and Service Delivery

• Provide direct nursing care and clinical oversight for clients receiving HPC and HPC+ services, including those with complex care and end-of-life needs
• Support care planning, clinical decision-making and escalation in accordance with organisational protocols and scope of practice

• Assist in quality improvement, audit and service evaluation initiatives within HPC and HPC+ services

• Uphold professional, ethical and regulatory standards in accordance with the Singapore Nursing Board requirements

• Bachelor's Degree in Nursing or equivalent recognised qualification
• Registered Nurse with the Singapore Nursing Board (SNB)
• Post-basic qualification or training in community nursing, gerontology, palliative care or leadership is preferred

Relevant Experience

• Minimum 4-6 years of nursing experience, with experience in community, eldercare or home care settings preferred
• Prior experience in a senior staff nurse, nurse clinician or supervisory role is an advantage
