• Answers all phone calls politely, and disseminates messages promptly when the staff is unavailable
• Assisting parents in locating their children in school, relay messages to classrooms, sending items from parents to children to the classrooms
• Providing necessary assistance to young primary school children such as contacting parents, sending birthday cakes up to classrooms etc..
• Assists the Directors in making appointments for meeting

Main duties and responsibilities:

Customer Service Coordination (70%)

· Responding to inbound enquiries (calls and emails) from SANS students, customers and community members including meticulous record keeping and follow-up to ensure successful and timely closure of all queries and requests.

· Providing assistance to students and customer organisations for payment of course registrations and reviewing event registration reports to help ensure timely payment of student accounts

· Processing student registrations and completing student administration tasks including updating student records, assisting with student account issues and providing invoices and other official documents to customers

· Assist with general administration and ad hoc tasks supporting the sales team

· Liaise with the marketing director for the arrangement of marketing collaterals for regional events such as exhibit, conference, roundtable discussion and Community night talks

· General ad-hoc duties as requested by management

Event Support (30%)

· Provide event support to regional events such as conferences and Community Night talks, as well as attend SANS trade events as required by the respective Directors

· Assisting with the execution of SANS Community Nights in assigned regions, including telephone invitations, confirmations to attendees from target client organizations and follow up with thank you notes post event. These marketing events may include event planning, event set- up, greeting attendees, staffing of the registration desk, distribution of marketing material, liaising with the venue staff and wrap-up activities

· Assisting with the internal events such as team offsites, workshops and other training events

· Be accountable for invoices/payments prior to events

RESPONSIBILITIES

The Reablement Care Coordinator serves as the central coordinator of the client's reablement journey, ensuring seamless integration across disciplines, programme phases and community resources. The role bridges healthcare, social care and community support systems to facilitate meaningful and sustainable outcomes for seniors.

Key Responsibilities

Care Coordination and Case Management

• Lead end-to-end coordination of the 12-week reablement pathway.
• Conduct intake triage, screening and enrolment of programme participants.
• Review Clinical Frailty Scale (CFS) levels and population health datasets to support participant selection.
• Consolidate interdisciplinary assessments into a unified reablement plan.
• Coordinate intervention schedules across multiple disciplines and stakeholders.

Outcome Monitoring and Programme Evaluation

• Monitor client progress using validated outcome measures including INTERAI, COPM and ASCOT.
• Identify barriers to progress and coordinate intervention modifications.
• Maintain accurate programme documentation and outcome records.

Multidisciplinary Team Coordination

• Facilitate case conferences and interdisciplinary reviews.
• Coordinate communication among therapists, nurses, social workers, therapy assistants and community partners.
• Ensure continuity of care throughout programme phases.

Community Reintegration and Sustainability

• Coordinate transition plans into Active Ageing Centre programmes, CARE Market initiatives, SEED programmes, volunteer opportunities and microjob pathways.
• Support development of sustainable participation plans beyond programme discharge.

Long-Term Follow-Up

• Conduct 6-month and 12-month follow-up reviews.
• Monitor for early signs of decline and facilitate timely reactivation of services where required.

• Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy or other relevant healthcare discipline.
• Minimum 2 years of experience in healthcare, community care or case management settings.
• Strong stakeholder management and communication skills.
• Experience coordinating care across multiple service providers.
• Proficiency in Microsoft Office applications
• Experience in community care, transitional care or active ageing programmes.
